Trailblazer provides 1-to-1 keyworker support for young people aged 16’24 who are economically inactive and want help to:
Improve mental health
Build confidence
Develop employability skills
Access training, education, volunteering or work
Support is tailored to each young person’s goals and barriers, with keyworkers offering practical guidance, emotional support and structured planning.

Services are via referral either on our website at swrmind.org.uk or over the phone on 01723 339838. We accept self referrals, referral from family members of any professional (consent must have been given for the referral.)
This service is not specific to SEN but would try to make adaptations.
This support is specifically for young people. There is also run a monthly parent peer support group for parents/guardians of young people with mental ill health or neurodiversity.
