The Humber and North Yorkshire Keyworker Service
Dynamic Support Keyworkers work with autistic young people and young people with a learning disability aged up to 25 who are on the Dynamic Support Register (DSR).
The DSR identifies individuals who are at risk — or may become at risk — of admission to a mental health hospital without the right support.
Keyworkers help by:
Ensuring young people and families get the right support at the right time
Listening and understanding what matters to the young person
Working with services to reduce barriers and achieve change
Creating a personalised plan with the young person and their family
Support is holistic, relational and focused on preventing unnecessary hospital admissions.
